Job description

We are seeking a dedicated and compassionate Occupational Therapist (OT) to join our team and contribute to improving mobility, function, and quality of life for our homebound clients. The ideal candidate will be patient-focused, results-driven, and passionate about promoting independence through personalized rehabilitation services.

Key Responsibilities:

  • Conduct evaluations and develop individualized treatment plans based on patient assessments.
  • Provide therapeutic exercises, mobility training, and pain management techniques in the home setting.
  • Educate clients and caregivers on home exercise programs and safe movement strategies.
  • Monitor and document patient progress, adjusting care plans as needed.
  • Collaborate with physicians, nurses, and other care team members to coordinate care.
  • Maintain accurate clinical records in compliance with regulatory standards.
  • Promote recovery and prevent further injury or decline through evidence-based practices.

Qualifications:

  • Current state licensure as an Occupational Therapist (OT).
  • Experience in home health or geriatric care preferred.
  • Strong clinical, communication, and time-management skills.
  • Ability to work independently and adapt to varied home environments.
  • Proficient in documentation and use of electronic health records.
  • Physical ability to assist clients with mobility and perform therapeutic interventions.
