Sometimes the road to recovery after a surgical procedure or traumatic event includes learning how to accomplish day-to-day tasks in a different way. HomeCare Crew Inc. knows that receiving Occupational Therapy in the comfort and familiarity of your own home makes the best use of your time. Think about it – why receive therapy in a clinic or rehab center only to re-assimilate yourself once you return home? The best course of action is to let our professional fully certified occupational therapists work with you in the setting where you will be spending the most time – your own home.
Occupational Therapists from HomeCare Crew Inc. are available to help you regain function for such activities as bathing, dressing, eating and more, putting you on the path to greater independence.
Each patient referred for occupational rehabilitation services will receive in-depth functional evaluations and assessments by a qualified therapist before treatment and education are initiated. Such assessments may include:
Functional assessment, including mobility and restrictions
Ability to perform ADL’s
Range of motion
Housekeeping and home maintenance, including home safety
Current functional level
After a comprehensive assessment our qualified occupational therapist develop an individual rehabilitation care plan. The patient/family is involved in the implementation of the rehabilitation care plan, including:
Identifying actions or interventions to achieve goals
Coordinating and collaborating with other healthcare professionals
Rehabilitation treatment plans and goals are individualized to each patient and designed to assist the patient in maintaining and/or achieving his/her optimal level of self- functioning, independence and self-care. Each patient will be reassessed every visit for progress toward goals and compliance with and implementation of the rehabilitation care plan.
