Each patient referred for occupational rehabilitation services will receive in-depth functional evaluations and assessments by a qualified therapist before treatment and education are initiated. Such assessments may include:

  • Diagnosis

  • Functional assessment, including mobility and restrictions

  • Ability to perform ADL’s

  • Range of motion

  • Housekeeping and home maintenance, including home safety

  • Current functional level

  • Independence

  • Measurable goals

After a comprehensive assessment our qualified occupational therapist develop an individual rehabilitation care plan. The patient/family is involved in the implementation of the rehabilitation care plan, including:

  • Identifying actions or interventions to achieve goals

  • Coordinating and collaborating with other healthcare professionals

Rehabilitation treatment plans and goals are individualized to each patient and designed to assist the patient in maintaining and/or achieving his/her optimal level of self- functioning, independence and self-care. Each patient will be reassessed every visit for progress toward goals and compliance with and implementation of the rehabilitation care plan.
